Those questions will be developed by the editor and the contributors. The idea is to provide a quick reference to the most current problems encountered with a specific issue developed in the chapter. Nowadays, the busy clinician is unlikely to exclusively rely on lengthy textbooks to learn specific complex techniques. That is one of the reasons for the popularity of CTO courses, live demonstrations, and conferences and, there are consequently many courses offered around the world to help new operators in the field.
