描述.This book provides detailed, up-to-date information on gastroesophageal reflux disease (GERD) and all aspects of laparoscopic antireflux surgery. Clear descriptions of surgical techniques and important surgical tips are combined with state of the art research results to create a unique resource for general surgeons and other health care providers. The book opens by presenting the latest knowledge on the pathogenesis of GERD, identifying the target symptoms, and explaining diagnostic tests and their interpretation. Failure and Revision of Antireflux Surgery,nclude hiatal herniation and wrap slippage..Revisional surgery, including re-do fundoplication and Roux-en-Y gastric bypass, is successful in over 80% of cases. However, patient satisfaction and rates of symptom resolution decrease with each successive reintervention, while the rate of complications increases.

GERD Pathogenesis and Essential Anatomic Knowledge for Antireflux Surgery, Cruroplasty and the wrap constructed by gastric fundus should not be too tight nor too loose. Antireflux surgery also decreases transient LES relaxation (TLESR) by decreasing absolute number, and the proportion of TLESRs that are accompanied by reflux.

Long-Term Outcomes and Cost-Effectiveness of Anti-reflux Surgery,ation..Numerous studies comparing the cost-effectiveness between the surgical and medical treatment for gastroesophageal reflux disease showed that anti-reflux surgery as a cost-effective alternative to long-term maintenance therapy with proton pump inhibitors.

Anti-reflux Surgery During Specific Situations: Achalasia, Para-esophageal Large Hiatal Hernia,per length of the intra-abdominal esophagus, crura repair, and anti-reflux surgery. Anti-reflux surgical procedure with or without Collis gastroplasty should be considered to reduce postoperative dysphasia and reflux.

Emerging Surgical Options,ctive trials, although studies comparing the efficacy, durability, and side effects of these procedures to laparoscopic fundoplication have not been conducted. EndoFlip measures the distensibility of a hollow organ across a balloon catheter and may act as an intraoperative objective measure of LES function.
